Yup, a pup. John and I have had big dog lust for years now. And so we have succumbed and are soon to be the proud owners of a blue-brindle Great Dane puppy. We both love the breed and I have actually grown up with these gentle giants so we are prepared for the 'counter surfing' that comes along with them. He is too little to come be with us just yet , but will arrive Mid- November.  Just in time for the Holiday madness. &lt;br /&gt;It's a 'he'.&lt;br /&gt;We are soliciting names. We LOVE the house.  We keep waiting for the "real owners" to come back and thank us for our house sitting services. It is truly wonderful. &lt;br /&gt;Of course there was the usual closing drama which included my being awake for 41 hours straight (I was post call), the movers arriving and the closing being scheduled simultaneously etc etc. But who cares! No more, my friends...&lt;br /&gt;&lt;br /&gt;On Thursday I finished work and walked outside of my clinic toward my car to head home.  This clinic is not in a great neighborhood and provides care to the under served of Baltimore. You get the gist. I inadvertently stepped in front of a man who had just acquired his lunch and apparently couldn't wait to get started on his fried lake trout sandwich. I excused myself, having stepped in front of his likely 300lb frame mid stride. He was wearing not the stock brokers business suit, but rather jeans that rode down to mid gluts, a wife beater t shirt that was short enough to allow me the pleasure of seeing his abdominal panus hang nearly to the level of the jeans. After letting me know this was not a problem for him (It ain't no thing, baby)he grinned, and stated emphatically: "It sure is a fine thing to see a REAL lady up in here. A FINE thing." The best part of all was that I had the pleasure of watching the lake trout being broken down in his mouth by the amylase in his saliva as he made this statement mid chew. Smacking. Swirling. Lake Trout. These are my best friends this weekend.  We are in the midst of packing hell and let me tell you, it is incredible the amount of unadulterated crap one can acquire over four years with young children.  This is compounded in our case because when we moved from NY to B'More we stored boxes full of more crap for the time when we would buy our own house again. Well guess where they all went yesterday after not being open for four years.  That's right.  My two best friends.  Goodwill and the dump.  I think five trips in all. Today is more packing/tossing.&lt;br /&gt;&lt;br /&gt;I also met one of our soon to be new neighbors yesterday. A very elderly, very adorable seemingly frail little old lady named Betty with a mighty firm handshake for someone who can't weigh more than 98 lbs soaking wet. We were over at the house doing a "drive by" (equals the Wrights stalking their new house) and she was in her doorway across the street peeking out at us curiously so we went and said hi. I think she may have cats. Nap. Haven't actually had one, but that's all I really care to do.  &lt;br /&gt;We are very fortunate to have friends in the neighborhood who recently made a move and gave us all their boxes.  This  means when I get home from work each day it's time to pack.  However, my mind can think of nothing but sleep.&lt;br /&gt;Most likely because i don't feel like I will be getting any next week.Monday is a night shift. Fine.  Thursday during the day we will likely close on the house, then it's another night shift immediately afterwards. Then Friday, we move. So, no resting in our cozy bed post call.  It will be being loaded on a truck.&lt;br /&gt;Sleep.... It started on Friday evening with a beautiful bouquet of roses and a box of chocolates delivered to my front door.  &lt;br /&gt;They are such beautiful vibrant colors and are starting to open up nice and full, now.   The next morning was breakfast in bed before piano lessons.  French toast. Presents  included cards and poems and plants.  And Juno.  I love that movie. It makes me pee from laughing. We had lunch out on Saturday which is always fun.  Sunday John made dinner and cleaned up the whole mess.  Now THAT is a big treat. The no cleaning deal on Mother's Day is key.  &lt;br /&gt;&lt;br /&gt;I am off today and therefore also refuse to clean, in favor of reflexology and a haircut. This means I am on call on Saturday.  We'll see how that goes. I am sure my pager will go off at 3:00am with such emergent issues as constipation and round ligament pain. Maybe there will be a nice birth. Time will tell.&lt;br /&gt;&lt;br /&gt;Chloe is to be in a talent show at school tomorrow.  She's singing "Good Morning Baltimore" from Hairspray.  I'll post it. It is certainly not to be missed.&lt;br /&gt;&lt;br /&gt;Amelia was accepted in to The Roland Park Advanced Academics Middle School.  She's a crackerjack. She'll be going.&lt;br /&gt;&lt;br /&gt;It also looks like we are buying a house. This week I was on L and D for 4 days in a concerted effort to get oriented quickly.   Most of the time it was very positive, but I did have one whopper of a  nightmare of a birth.  I won't get into it in a post due to my observance of HIPPA regulations, but suffice it to say, it made me question my career choice.  I am getting over it however .  I was back there yesterday and the absolutely amazing women I work with were fantastically supportive. Word on the street is that as a new grad midwife in this type of institution I am likely to feel incompetent for about a year. Good times.&lt;br /&gt;My colleagues had a "Welcome to the Practice" party this week as well.  Very fun.  Very sweet. &lt;br /&gt;Girls are both wicked sick.  Chloe has bilateral ear infections that kept her home one day last week. Amelia wound up in the ER last night with a flu so bad she needed to be rehydrated. Poor babies.&lt;br /&gt;Guinness is getting bigger and more terrible every day.  You know the dog from the Simpsons, Santa's Little Helper? We set up the crate at night time in the downstairs bathroom for Guinness to sleep in. Everything we read said not to take the puppy out of the crate no matter how much she barked. (Yes, we were Ferberizing our dog.) First night: 45 minutes of barking.  Second night: 1 hour of barking. Third night: an hour and 15 minutes of barking and an intervention. We brought the crate into our bedroom.  She immediately calmed down and went to sleep.  We've had no trouble since.  She is becoming very happy to bark at Hattie and try and engage her in a duel.  Hattie has none of it.&lt;br /&gt;&lt;br /&gt;Bigger news that the puppy update:  I have privileges!! That's right.  I can officially  see patients in the office, catch babies and write prescriptions. I can bill sometime next week. WOOHOO!! &lt;br /&gt;Tomorrow is my first official L and D shift, even though it's only four hours. Guinness is a sweet little blackish-brown 12 week old retriever mix that we rescued from the animal shelter today.  We couldn't resist her cuddles or her kisses or her puppy belly. Guinness has one bright blue eye like you'd find on a Husky.  Her other eye is half blue and half brown.  She's a bundle of yum.&lt;br /&gt;Millie has perked up a bit with a new friend in the house and seems to be embracing her role as mentor.  Let's hope she leaves out Barking 101.  Hattie Cat is more curious that anything.  Merlin is a general bastard, hissing and growling and puffing out his tail.&lt;br /&gt;We are crate training her, which is a new endeavor for us. Let me first say to everyone out there whether you have anything to do with women's health care or not: Read this book. Especially if you are considering having a baby. &lt;br /&gt;Ok, so Ms. Block went on to discuss the usual cesarean rate, rising maternal mortality rate in the US,VBAC bannings etc. Then her last 'painful truth' was this:"Where is the Outrage?" Why aren't women in the US absolutely fit to be tied that these things go on and why aren't we clamoring to do something about it? &lt;br /&gt;Now this was ever so slightly touched on by the briefest of comments but it is a notion that begs more attention and which I have been thinking about ever since.  Where is the feminist movement?  Why  is birth, the most basic of feminine function not addressed as a reproductive rights issue?  We put all our eggs in the pro-choice basket, and yes I see how at risk we are of losing our right to choose and how important that is, but I truly believe that women attempting a VBAC have the right to refuse a cesarean which is "required" simply because that hospital in which their provider practices has banned VBAC's. It boggles the mind how bright, intelligent women who would never consider investing some money without first doing all her research, reviewing the prospectus and weighing all the financial risks and benefits, simply throw the fate of their bodies and their babies into the hands of their care provider without taking the time to research what will happen to their bodies with that particular care provider. &lt;br /&gt;The bottom line is, that despite all our forward momentum for women's rights,some of us still fundamentally believe that our bodies are lemons and that we need saving. &lt;br /&gt;We need to work on this.  Women need education.  Not the kind where we sit in class on a mat learning relaxation techniques. The kind where you turn on the TV and you see Barbara Walters  showing us exactly what the risks of cesarean are when it is unnecessary. The kind where after watching it you turn to your partner on the couch and say, "Oh, my god, is that true?  Does that really happen?" and you go out and read a book.  Women should not have to do scholarly research to find this stuff out.  It should be mainstream. I am holding it in my hot little fingers.  This means I can actually sign charts, deliver babies and (maybe, I'm not sure) write prescriptions on my own. As an added bonus the credentialing committee meets Wednesday and so I will be truly all set to go. &lt;br /&gt;The ironic piece of this is that today was the hearing for HB 1407 Birth Options Preservation Bill in Maryland. That's bill to prohibit the Board of Nursing from requiring the written practice agreement in the first place. When John took her the other day she used the knee lock and butt drag technique.Yesterday, she gave me the same forlorn look over her shoulder which warranted her a 'get out of jail free' card from the pound to our little apartment in NYC 12 years ago. Sadly, for Millie, we are heartless and through the doors she went.&lt;br /&gt;Still waiting on the letter from the BON.  Still can't catch babies.&lt;br /&gt;The days of my intended incredible orientation that had been so generously arranged for me are ticking way, not being used appropriately. &lt;br /&gt;Sigh. When a midwife, or any advanced practice nurse, wants to hang out their shingle and go to work, the board which governs their practice, in this case,the Board of Nursing, requires then to have a written agreement with their collaborating/consulting physician. A series of meetings take place about six weeks  after that agreement hits the BON.  Then a letter is sent to the midwife/AP nurse, and only then, may she function outside of the RN role. This translates into months ticking by after passing boards during which you must sit on your hands and not be able to orient fully to your new job.   &lt;br /&gt;While this seems like maybe it would be a benign little document there are a number of problems with it other than the one listed above.   First, it's a supervisory document which is superfluous for those licensed professionals who have passed their board exams in their field of expertise and demonstrated competency.  Second, it drives up the malpractice insurance for that physician with whom the AP nurse or midwife is collaborating.  It is for this reason, and, frankly, a financially understandable one, that physicians often will not have a collaboration with a midwife.  It is also for this reason that many midwifery practices wind up closing.  No one can afford to be their collaborating physician.   &lt;br /&gt;&lt;br /&gt;This greatly limits a woman's right to choose her caregiver when she is seeking well woman,family planning,  intrapartum and postpartum care. Women deserve the right to choose their caregiver.Why on earth would a ursing organization seek to limit that choice?  &lt;br /&gt;There is a bill in the MD state congress right now to prohibit the BON from requiring a written practice agreement, as so many other states have done. If you live in the state of Maryland, please do take the time to write or call your legislator and ask for their support on HB 1407. So far so, good, but I honestly haven't had to do a lot of caring for clients yet.  I still have a number of things to do before being fully credentialed at the hospital.  Certain classes about billing and coding, conscious sedation (huh??) and malpractice insurance.  The good news is that the state Board of Nursing finally has recognized my license.  It won't be long now.  &lt;br /&gt;I am loving working with the CNM's I am working with.  They are all brilliant women at different levels of experience, and in their lives who care deeply about the midwifery philosophy and the families to whom they provide care.  I am lucky new midwife.   &lt;br /&gt;I spent some time on Labor and Delivery yesterday.  I forgot how much I'd missed being there.  Even though I couldn't really touch anyone it felt completely like I belonged right there. Looking forward to next week.  Love it.&lt;br /&gt;&lt;br /&gt;But for today..... it's a house filled with 10 year old.  One of the beans is having a birthday today. We had in our home some pink eye (check),a sliced open finger (check), a broken sewage pipe at the kids school(check) followed by two vomiting children (check, check). All on separate days.  While one of my three coveted weeks off was completely shot by this, I figure pretty much everything that could happen to keep me home has now officially happened.  So I'm thinking I should be good for the rest of the school year. &lt;br /&gt;&lt;br /&gt;I start work in about a week. While it's a little bit nerve racking I am very excited to get started. Things are getting all lined up.  I have my ID's- one for the hospital, one for the school.  I have a pager number.  I have an office.  I have a schedule.  I have my babysitters. Let's hear it for Virchow's Triad: epithelial injury, hypercoagulability and VENOUS STASIS. My big girl Amelia wants desperately to be a writer when she grows up.  Here is the poem she wrote for her teacher at Christmas. My Nurse-Midwifery Board Certification exam is 10 days away. The more I study the more I waffle between feeling like my program covered a very small percentage of what I am expected to know to feeling like the topics I focused on for comps are merely review. That part is good.  Confounding the studying issue is the pressure of keeping my job if I pass, and the offer being withdrawn if I don't.  175 multiple choice questions. On the spot results.&lt;br /&gt;When I compare it to what the midwives in Colonial Williamsburg had to go through with growing and grinding their own herbs, creating their own tinctures, walking or riding on horseback in inclimate weather at all times of day and night, walking in on who knows what type of labor it puts things in perspective. The wealth of resources I have available for studying and the sweet little 12 hour shifts I'll be working two miles from home are a far cry from the demands of our foremothers in midwifery.  &lt;br /&gt;Hoping to keep all those things in mind when I feel like griping as the exam draws nearer.&lt;br /&gt;&lt;br /&gt;Note the pic which shows a typical 1774 Apothecary.  Gentlemen of the day trained primarily via apprenticeship, and were meant to function not only as pharmacist, but physician, dentist, and surgeon.  Midwives might come by here and purchase some supplies needed for their birth kits, for example Borax which was intended to hasten a slower labor. So I thought I'd talk a little about what a midwife actually does.&lt;br /&gt;&lt;br /&gt;Midwives are highly trained professionals who provide primary care, gynecological care, preconception care, prenatal, intrapartum and postpartum care, family planning,and menopausal care. Midwives listen to women. When you see a midwife you become a partner in and take ownership of your own health care. &lt;br /&gt;We see clients in the office or clinic and work on labor in delivery units, birth centers or at a client's home.&lt;br /&gt;There are many different types of training to become a midwife.  CNM's are midwives who previously obtained nursing degrees. Most CNM's entering the field now are masters trained, although not all. CM's receive the same training as CNM's minus the nursing training. Many states recognize the CPM (Certified Professional Midwife) These midwives train through a different path and are also highly qualified healthcare professionals. &lt;br /&gt;My training is as a nurse midwife.  Once I pass my boards, I will be a CNM. We began the day with a Blessing of the Hands ceremony. This is a non-denominational service at which the midwife graduates are specifically honored and our hands blessed by other midwives.  One of my very favorite preceptors did the honors with me. I say with because it is a priest that does the actual water dumping.  There were a few poetry readings, and candles passed, some beautiful classical guitar music and just an overall feeling of amazement as we received our certificates of completion.  One of the especially special things we did was to travel to a town outside of DC to get henna painted on our hands.  There were just a few of us, and it was a very special time.  My girls each got a hand done as well- so cute!&lt;br /&gt;&lt;br /&gt;Then we moved over to the big auditorium where we sat hungrily on line waiting for our masters awards.  This was done in conjunction with the other NP disciplines at Georgetown: Acute Care NP, Family NP, and Nurse Anesthesia.  It was complete with trumpet fanfare.  But no actual diplomas.  Apparently they come out once a year, in the spring, so I will have to wait for that bad boy to hang on my wall. &lt;br /&gt;&lt;br /&gt;It has all felt incredibly surreal up until this morning when I received my schedule for work!  It looks like an extremely kind orientation. You eat, sleep and breathe baby catching. But as of this past weekend it is over! I have graduated with my MS and will soon be starting my first midwifery job, in February at a local  midwifery practice. It is a faculty practice at a nearby University which brings added teaching responsibility but I  am looking forward to the challenge.  (Just as soon as I pass my boards!) What an amazing experience Friday and Staurday were.  I arrived at the hospital on Friday at 7:00am and left at Saturday morning at 8:00 am.  I delivered a baby at 12:01 am and 6:39 am.  I got one hour fo sleep inthe call room the entire time.  I was with one of my favorite preceptors and even thought it was a prety long and drawn out evening it was an amazing learning experience with a sick baby and then a really big baby.  I got to basically manage both patients all day/night long on my own and do the deliveries. Totally amazing.  And exhausting.  It's Sunday and I am still feeling like I need a nap. Yesterday, they wanted to go to the NAL meet and watch their school team.  (NAL= National Academic League) It was great to be able to be spontaneous with them and not have to wiggle the schedule this way or that to make a simple request like that work out. They really enjoyed it, despite the fact that Midtown got trounced upon pretty severely by Roland Park. I think today we will get some CREPES.  And I think it will be a SURPRISE! YUM!&lt;br /&gt;John has choir tonight so we will get some extra time together.&lt;br /&gt;Studying is going much better today.  Pharm reigns rather than procrastination. Not so, dear friends.  I am currently suffering from a cold and desire nothing more than sleeping with the cats. I am taking the morning off.  This means I have exactly 30 minutes of procrastination time left. &lt;br /&gt;I was over at the Hopkins Med campus this morning purchasing a book to aid in my pharmacology plight, and became a little nostalgic for the place. Hard to believe that's possible!&lt;br /&gt;This week is "Spring Break", after all.&lt;br /&gt;We are all heading to NY this weekend to help my uncle celebrate his retirement.  Then we are visiting  Aunt Kim on the way home.
